Home Healthcare Services

Our Home Health Care Services are open to receive referrals. We work with families and follow physicians' orders to bring quality home health care to the home. With the support of our dedicated staff, we are prepared to provide your patient the support and quality of care they need to recover and heal at home.

Utilizing Senior Lifestyle Solutions Home Healthcare enables an individual to receive highly effective medical treatment, while remaining as independent as possible in the comfort of their own home.

A registered nurse (RN) or a licensed practical nurse (LPN) will be assigned to set up and manage the patient's care plan, with consultation from the primary physician. The nurse will perform an initial health assessment and continue to monitor the patient's vital signs and overall health. The nurse will also provide education to the patient, their family and caregivers on their condition and treatment. Other services that may be provided include:

Physical therapy focuses on improving the patient's ability to move their body following an illness or injury. Therapists work with patients on strength, mobility, flexibility, coordination and balance​. Physical therapy often helps to decrease the duration and severity of disabilities that arise from chronic disease. Physical therapists may provide these services or help with these conditions:

Speech-language pathology services (SLP) can boost a person's ability to communicate and to swallow. SLP addresses the declines associated with the aging process, such as neurological difficulties, age-related illness, and deterioration of the swallowing mechanism. Occupational therapists may provide these services or help with these conditions:
  • Dysphagia
  • Aphasia
  • Dysarthia
  • Apraxia
Breathing problems due to:
  • Lung diseases
  • Tracheotomy
Cognitive-linguistic impairment treatment:
  • Restoring memory
  • Sequencing

​Complete or partial loss of voice
Payment Options
Medicare
  • ​Covers eligible home health services like part-time or "intermittent" skilled nursing care, physical therapy, and occupational therapy services.
  • To be eligible, you must:
    • ​​Be under the care of a doctor, and you must be getting services under a plan of care created and reviewed regularly by a doctor.
    • Need, and a doctor must certify that you need, one or more of these:
      • ​​​​​Intermittent skilled nursing care (other than drawing blood)
      • Physical therapy,  or continued occupational therapy services
        • ​Your condition must be expected to improve in a reasonable and generally predictable period of time.
        • You need a skilled therapist to safely and effectively make a maintenance program for your condition.
        • You need a skilled therapist to safely and effectively do maintenance therapy for your condition.
        • The home health agency caring for you is approved by Medicare (Medicare Certified).​​​​
    • ​You must be homebound, and a doctor must certify that you are homebound.​
  • You are not eligible for the home health benefit if you need more than part-time or "intermittent" skilled nursing care.

U.S. Department of Veterans Affairs (VA) comprehensive health care benefits through the Veterans Health Administration (VHA)
  • In-home care is part of this standard medical benefits package.
  • Must meet three eligibility requirements
  • Homemaker Services and Home Health Aide Services
  • Only home care companies in the VA's network can provide these covered services. Senior Lifestyle Solutions is a Department of Veterans Affairs Community Care Network (CCN) Participating Provider.

Long-Term Care Insurance
  • Senior Lifestyle Solutions invoices our services every two weeks for the preceding 2 week period. The invoice can be sent directly to the client, a family member responsible for the clients finances,  a trusted advisor, or long-term care insurance provider.
  • We can assist in filling out claim forms and other paperwork to put your long-term insurance benefits to work as quickly as possible.
  • We can provide detailed service invoices directly to and often required by insurance providers.

Private Pay
  • Senior Lifestyle Solutions invoices our services every two weeks for the preceding 2 week period. The invoice can be sent directly to the client, a family member responsible for the clients finances, or a trusted advisor.
  • Invoices can be paid for by check or bank transfer.
