The transition home from a hospital or rehabilitation facility after a serious injury or illness can be confusing. ALS care
Amyotrophic Lateral Sclerosis (Lou Gehrig's Disease) is a progressive neuromuscular disease. It attacks motor neurons in the brain and spinal cord which transmit signals to the voluntary muscles throughout the body. When motor neurons die as a result of ALS, the ability of the brain to control muscle movement is lost. Weakness spreads to all the voluntary muscles and progresses to paralysis.
ALS does not affect sensory nerves so there is no loss of feeling in the paralyzed areas. ALS only affects motor pathways in the brain so awareness, thought and intelligence are not generally affected even after paralysis makes it impossible for the person to communicate.

MS care
Multiple Sclerosis is a chronic disease that affects the central nervous system, which consists of the brain, spinal cord and optic nerves. MS gradually destroys myelin, which is a protective coating covering nerve cells that act as a conductor in helping send signals at high speeds from nerve cell to nerve cell. Without myelin, the brain and spinal cord can't communicate with the nerves in the rest of the body. This can cause problems with muscle control and strength, vision, balance, and sensory issues. As it progresses, symptoms may include fatigue, difficulty speaking and swallowing, bladder dysfunction, inability to think clearly, and depression. The course of MS varies from person to person. Some have minor issues while others become seriously disabled, with most being somewhere in between.

Without effective management, diabetes can spiral into a wide variety of complications like nerve damage, kidney damage, eye damage, stroke, heart attack and atherosclerosis.

A stroke is caused when the blood supply to an area of the brain is cut off, depriving the brain cells of oxygen and causing them to become damaged or die. Early detection is crucial in minimizing the affects a stoke can have. If not immediately treated, a stroke can cause permanent damage to the brain causing impaired speech and motor skills. Following the stroke, the brain cannot regrow damaged cells, however new paths will form in the brain to reactivate living cells that become disconnected. The process is slow and requires a variety of therapies to regain the use of damaged regions. Stroke prevention includes monitoring blood pressure, with high blood pressure being the largest single risk factor, along with a healthy diet and exercise.
